There Are Multiple Ways to Pay for Services
Insurance
Insurance is accepted for all services offered. If you choose to use insurance for payment, you will be responsible for any co-pays at the time of your visit. You will also be responsible for any co-insurance due after insurance has reviewed your bill and paid their portion. The remaining balance after insurance is due upon receipt of the bill. Our office is happy to bill your insurance plan for you.
Cash Pay
Choose Cash Pay when you book your appointment and pay at the time of your visit to receive a discount. This option is only available for those without insurance or when insurance doesn’t cover specific services (like functional medicine).
Your Cash Pay Visit:
1. Make an appointment: Schedule your visit and secure it with a credit card.
2. Complete intake forms: Fill out and submit the required forms before your visit.
3. Visit: Meet with your provider to discuss your medical needs.
4. Pay: Payment is due at the time of service to receive the discounted rate. Failure to pay at the time of service will result in a bill for the full cost of services, due upon receipt.
5. Receive a prescription (if needed): If a prescription is necessary, it will be sent to your chosen pharmacy.
Fee Schedule:
Initial Appointment: $200
Follow-Up Appointment: $100
Note: Both initial and follow-up appointments are discounted. Cash Pay applies to all listed services.
